- Sage Health Services will verify insurance eligibility and will inform family of coverage specific to applied behavior analysis services.
- Obtain a referral / prior authorization (if required)

- If required, Sage Health Services will obtain a required Prior Authorization through your insurance carrier
- This process typically takes a 3-5 business days in order to receive a response from insurance carriers.
- Complete intake and assessments

- Complete any intake paperwork (consent forms, release forms, insurance forms) via DocuSign
- A Sage Health Services BCBA will complete a comprehensive assessment (VB-MAPP, ABLLS, PEAK, functional behavior assessment, skills inventories, caregiver interview).
- Participate in caregiver interviews — your input is essential for goal setting.
- Develop the individualized treatment plan

- The BCBA creates a written treatment plan with: target goals, session frequency/duration, therapy setting, intervention methods, and data collection procedures.
- Review the plan carefully — ask for measurable objectives, baseline data, and how progress will be reported.
- Sign consent for services.
- Secure authorizations & schedule services

- Sage Health Services will submit the treatment plan to your insurer if prior authorization is needed. Provide any additional requested documentation.
- Once authorized, arrange schedule: session days, times, and location
- Start therapy & establish routines

- Attend scheduled ABA Sessions, including parent training as set up.
