About Us

Sage Health Services was established in 2011 to help improve the lives of the people we support. At Sage Health Services, we believe that every child and adolescent diagnosed with autism is unique and in need of supports to lead them on their path to success. Each case therefore starts out with an initial assessment conducted by a qualified Board Certified Behavior Analyst (BCBA). This assessment will identify strengths and deficiencies and help in creating a treatment plan that focuses on these findings. As progress is made, treatment plans are frequently updated to align with a client’s current needs and challenges.

Jared Shoemaker - Chief Operations Officer

Jared Shoemaker has dedicated more than two decades to serving individuals with disabilities. Since beginning his career in 2001, he has dedicated his professional career to ensure that individuals with disabilities have access to quality services that make a lasting impact on their lives.  

Jared earned his Bachelor of Science in Psychology from Brigham Young University and his Master of Arts in Criminal Justice from the University of Nevada, Las Vegas. His educational background, combined with years of hands-on experience, has provided him with a comprehensive understanding of human behavior, program development, and effective support strategies. 

In his spare time, Jared enjoys travelling and learning about new cultures, reading about current events and most importantly, spending time with his wife and two teenage children. 

Nate Boyack - Chief Financial Officer

Nate Boyack has been working in the Developmental Services field since 1998. He has over 25 years of supervisory experience developing and running organizations that promote self-reliance and individuality. Nate has spent extensive time helping clients and company team members reach their full potential.

Nate has a BA degree from Brigham Young University and an MBA from the University of Nevada, Las Vegas. As part of his work with people with developmental disabilities, Nate served three terms as the president of the State of Nevada Association of Providers. He has worked with the Nevada Legislature during multiple sessions advocating for and promoting programs for his clients.

Nate is a Northern California native, and he and his family have lived in Las Vegas since 2002. In his spare time, Nate enjoys spending time with his family and volunteering at his church. He also loves sports and the outdoors and especially loves doing any combination of both with his wife and 4 children.

Danyelle Lincoln - State Director of Autism Services

Danyelle Lincoln is a Board Certified Behavior Analyst with over 10 years of experience in ABA, clinical management, and staff training. Danyelle has a BA in Research Psychology and a Master of Arts in Behavior Analysis from the University of Nevada, Reno. In addition to her role as Clinical Director for Sage Health Services, Danyelle is also a professor at Truckee Meadows Community College where she teaches psychology, behavior analysis, and sociology.

Danyelle is passionate about continuous learning and professional development, and she aims to advance her expertise in behavioral health, contribute to the growth of clinical systems, and support the development of both clients and SHS staff.

When Danyelle isn’t working, you can find her building coordinated holiday light shows for her home or tending to her chickens.

Jazmin Perez Fuentes

Jazmin Perez Fuentes is a Board Certified Behavior Analyst providing ABA services and clinical supervision at Sage Health Services. With over 9 years of experience working with children and adolescents in the field of behavioral analysis, Jazmin's approach is compassionate and centered on personalized and evidence-based care to help individuals achieve lasting and meaningful results.

She has experience providing direct behavioral analytic services in clinical and at-home settings, as well as collaborating with community partners, parents, and various treatment teams. Jazmin is passionate about both client and staff development and stays current with best practices in the field.

Jazmin initially joined Sage Health Services in 2017 and has served as the Associate Clinical Director as of 2025. She received a Master’s in Special Education from Arizona State University, and a Bachelor's of Human Development and Family Studies and a minor in Spanish Translation from the University of Nevada, Reno.

In her free time, Jazmin loves to spend time with her 2 children and husband, as well as baking and crafting.

 

Michelle Hunt

Michelle Hunt is a Board Certified and Licensed Behavior Analyst with more than a decade of experience spanning clinical education, disability services, and organizational training. Throughout her career, she has supported people across the lifespan—from early intervention to adult behavior programs—while leading treatment planning, compliance reviews, and the supervision of emerging practitioners. In addition to her clinical work, Michelle has held leadership roles focused on building effective training systems, strengthening organizational performance, and designing engaging curricula.

Her collaborative research and writing have appeared in several professional texts, including The Oxford Handbook of Acceptance and Commitment Therapy and the Journal of Sport Psychology in Action.

When she’s not working, Michelle enjoys creative writing, distance running, and Olympic weightlifting. She and her husband spend much of their free time hiking and exploring with their dogs, Holly and LePea.
