Referrals

To begin the referral and intake process, parents or guardians must complete the required intake information. Please use the link below to submit the necessary details. Be sure to select the correct link based on the client’s location and preferred language.

As part of the referral process, you will also be asked to upload supporting documentation, including proof of insurance and diagnostic records.

If there is no current availability for services at the time the referral is completed, the child will automatically be placed on our waitlist and contacted as openings become available.
